Shows fill very quickly! Be sure to enter early and complete your entry (see below for details) in order to be confirmed as entered and not incur the $50 incomplete entry fee.

All signatures (rider, trainer, owner, coach, parent) must be filled out and in black/blue ink
- signed federation entry form
- signed USEF waiver and release of liability (if showing both days you can sign one waiver, but you must write both show dates on it)
Vaccination record (in accordance with USEF GR 845) must be submitted with entry
Negative coggins within 12 months of show date must be submitted with entry
Emergency number must be listed
USEF & USDF membership cards (use eqverification.org) for horse, rider, owner, and trainer (and coach, if applicable) must be submitted with entry (Note: horse show office automatically generates membership cards, but you are responsible for making sure that they are all present and correct)

Additional documents required, if applicable:
- For MFS, must include a copy of a 63% at the highest test of the level (60% for FEI)
- For Para, must include copy of the official dispensation certificate
- For leased horses, proof of lease agreement on file with USEF and USDF
